Any insights into how the customer could package the CD to get it noticed?
Less is more.  I'd use a big, readable font that says something like "Joe Blow's aircheck and resume."  PDs are very busy and the more stuff a prospective employee hits them with, the less likely they are to feel that they have time to review it at a moment of opportunity. 

The key is to make it look really easy and quick for the PD to review it.  

What about slapping a station label on the mailer?
That's a good idea.

Would a CD in a slim case be better than a sleeve?
I think the case would be preferable to protect the CD, plus it might make your service feel a little more upscale. 

How do most applicants send an aircheck? mp3 on CD, playable in car, email, or posted on the web?
Most of the stuff I get is on CD now.  A few people e-mail mp3s or worse, links to a web site.  For some reason I never get to that stuff as quickly.  Out of sight, out of mind I guess.
